Can Appendicitis Be Treated With Antibiotics Instead of Surgery?

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

You may have read that appendicitis can now be treated with antibiotics instead of an operation. It's true for some patients — but it's not the right choice for everyone, and it comes with a real chance of the appendicitis returning.

Quick answer: in selected adults with uncomplicated appendicitis (confirmed on a scan, no burst, no abscess and usually no appendicolith), antibiotics can settle the attack. However, a substantial share — roughly 3 in 10 or more — need their appendix removed later. Laparoscopic appendectomy remains the standard, definitive treatment.

What research shows शोध क्या कहता है

Large clinical trials in Europe and the USA compared antibiotics with surgery for uncomplicated appendicitis. Antibiotics settled the first attack in most patients, but a considerable number had appendicitis again and needed surgery within months to a few years. Patients with an appendicolith (a small hard stone in the appendix) were much more likely to fail antibiotics.

Who may be suitable

  • Adults with uncomplicated appendicitis confirmed on ultrasound or CT.
  • No appendicolith, no abscess, no perforation.
  • Patients with high surgical risk, or who strongly prefer to avoid surgery after understanding the risks.

Who should have surgery

  • Burst appendix, abscess or signs of spreading infection.
  • Appendicolith on the scan.
  • Those not improving within 24–48 hours of antibiotics.
  • Many older patients — a mass seen on scans needs full evaluation.

The honest trade-off: antibiotics may avoid an operation now, but you live with a real chance of another attack — possibly at an inconvenient time or place. For most patients, a keyhole operation with a 1–2 day stay is the simpler long-term solution.
