Can an Anal Fistula (Bhagandar) Heal Without Surgery?

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

It's natural to hope a fistula can be treated with medicine alone. Unfortunately, an established anal fistula almost never closes permanently on its own. Understanding why helps you avoid years of cycles of antibiotics and discharge.

Quick answer: an established anal fistula rarely heals permanently without a procedure, because the tunnel is lined with tissue and remains connected to the anal canal. Antibiotics can calm an infection temporarily. Modern procedures — including laser (FiLaC), VAAFT, seton/ksharsutra and fistulotomy — treat the tunnel with different levels of invasiveness.

Why medicines don't close a fistula

  • The tunnel's inner lining becomes like skin; the body treats it as a normal passage.
  • The inner opening stays connected to the anal canal, so stool bacteria keep entering.
  • Antibiotics only reduce the current infection — once stopped, pus collects again.

What about home remedies?

Turmeric, herbal pastes and oils may soothe the skin but cannot close the internal opening. Delaying treatment allows the fistula to branch into new tracks, which makes the eventual treatment more complex.

Exceptions

  • Some fistulas linked to Crohn's disease are managed mainly with medicines, sometimes with a seton.
  • Tuberculosis-related fistulas need anti-TB treatment.
  • Very rarely, a recent small track after an abscess may close on its own.

These are diagnosed by the surgeon, sometimes with an MRI or biopsy.

Good news: "surgery" for a fistula today often means a minimally invasive, day-care procedure — not a large open wound.
