Why Does a Fistula Come Back After Surgery — and How to Prevent It
Many patients come to the OPD with a fistula that has been operated on once, twice or more. It's frustrating, but recurrence usually has an identifiable reason — and identifying it is the key to a lasting cure.
Quick answer: fistulas usually recur because a branch or the internal opening was missed, the fistula is complex or high, the chosen method did not suit the anatomy, or there is an underlying condition like diabetes, Crohn's disease or tuberculosis. Careful mapping (often MRI), the right method and good aftercare reduce the risk.
Common reasons for recurrence
- Missed internal opening or branch — the hidden part keeps the infection going.
- Complex or high fistula treated like a simple one.
- Method not suited to the anatomy — e.g., a sealing technique for a very branched track.
- Uncontrolled diabetes slowing healing.
- Underlying disease — Crohn's disease or tuberculosis.
- Poor wound care — wound skin closing on top before the depth has healed.
How recurrence is reduced
- MRI fistulogram before re-operation to find every track.पहले MRI।
- Examination under anaesthesia to confirm the internal opening.
- Tissue sent for testing where TB or Crohn's is suspected.
- Staged treatment for complex tracks (seton first, then closure).
- Sugar control before and after surgery.
- Regular dressings and follow-up until fully healed.
A recurrent fistula is not a hopeless fistula. With the anatomy properly mapped, most recurrent fistulas can be treated successfully.
What you can do
Keep all follow-up visits, do sitz baths, keep stools soft, control sugar, and report any new discharge or swelling early. Bring your previous operation notes and MRI reports to the consultation — they help plan the next step.

Frequently asked questions
Why does fistula come back after surgery?+
Most often because a branch or internal opening was missed, the fistula was complex, or healing was affected by diabetes, Crohn's or TB.
Can a recurrent fistula be cured?+
Yes. With careful mapping (often MRI) and the right method, most recurrent fistulas can be treated successfully.
Should I get an MRI before a second fistula surgery?+
An MRI fistulogram is usually recommended for recurrent fistulas to map all tracks before surgery.
Does diabetes affect fistula healing?+
Yes. High sugar slows healing and increases infection risk, so good sugar control is important around surgery.
