Anal Fistula Symptoms: Pus Discharge, Boil Near the Anus and Recurring Pain

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

A painful boil near the anus that bursts, leaks pus, seems to heal — and then comes back weeks later. If this cycle sounds familiar, you likely have an anal fistula (bhagandar). It's a tunnel, not a boil, which is why it keeps returning.

Quick answer: an anal fistula is an abnormal tunnel between the inside of the anal canal and the skin near the anus. It usually follows an abscess. Typical symptoms are a small opening that leaks pus or blood-stained fluid, staining of underwear, itching, and repeated painful swelling. It does not heal permanently with antibiotics alone.

Common symptoms

  • A small opening or pimple-like spot near the anus that discharges pus or fluid.
  • Staining of underwear with pus, blood or a bad smell.
  • Repeated painful swelling that eases once it bursts.
  • Itching and skin irritation around the opening.
  • Pain on sitting or during stool in some patients.
  • Fever when a new abscess forms.

How does a fistula form?

Small glands inside the anal canal can get blocked and infected, forming an abscess (a pus collection). When the abscess bursts or is drained, a tunnel may remain between the gland inside and the skin outside — that tunnel is the fistula. About a third or more of anal abscesses go on to form a fistula. See abscess vs fistula.

Who is more at risk?

  • People with previous anal abscess.
  • Diabetes.
  • Inflammatory bowel disease (Crohn's disease) and, in some areas, tuberculosis.
  • Long sitting jobs and poor hygiene may add to discomfort, though the root is gland infection.
Why antibiotics don't cure it: antibiotics can calm infection for a while, but the tunnel remains lined with tissue and reconnects to the gut. So symptoms return. The tunnel needs treatment.

When to see a surgeon

See a surgeon for any discharge near the anus that lasts or returns, and urgently for a hot, swollen, very painful lump with fever — that may be an abscess needing drainage. The existing article भगंदर क्या है explains more in Hindi.
