Stapler vs Laser Surgery for Piles: Which Is Right for You?

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

Patients often come to the OPD having read about "stapler surgery", "laser surgery" and "open surgery" for piles and are unsure which is best. Each has its place. The right choice depends on the type and grade of your piles — not on which sounds most modern.

Quick answer: laser treatment suits most grade 2–3 and selected grade 4 piles, with minimal pain and quick recovery. Stapler surgery (MIPH) is useful for larger circumferential prolapsing piles. Open surgery is still best for some large external piles and skin tags. Your surgeon chooses after examining the grade and type.

The three main options

Laser treatment (laser hemorrhoidoplasty)

A thin laser fibre is placed into the pile, and controlled energy shrinks it from within. There is no large cut or stitch, so pain is minimal. ||

Stapler surgery (MIPH / stapled hemorrhoidopexy)

A circular stapler removes a ring of tissue above the piles and lifts the prolapsing piles back into place. Useful when piles are large and all around.

Open surgery (hemorrhoidectomy)

The piles are cut out. It is the oldest method and very effective for large external components, but recovery is the most painful. ||

Which one is right for you?

  • Mostly bleeding piles, grade 2–3: laser is usually ideal.
  • Large piles prolapsing all around: stapler, or laser combined with lifting (pexy) stitches.
  • Large external piles or hanging skin tags: some excision may still be needed, often combined with laser.
Many patients don't need a single "pure" technique. A good surgeon combines methods to treat each pile in the best way in one sitting.

Questions to ask your surgeon

  • What grade are my piles, and why do you recommend this method?
  • Is there a fissure or fistula too, and will it be treated at the same time?
  • How many days off work should I plan?
  • What can I do to prevent recurrence?

Frequently asked questions

Which is the best surgery for piles?+
There is no single best surgery for everyone. Laser suits most grade 2–3 piles; stapler and open surgery suit certain larger or external piles. The choice depends on examination.
Is stapler surgery painful?+
It is usually less painful than open surgery, as the staple line is in an area with few pain nerves, though mild discomfort is common for some days.
Can laser treat grade 4 piles?+
Selected grade 4 piles can be treated with laser, sometimes combined with other techniques. Very large or fibrosed piles may need excision.
Which method has the least recurrence?+
Recurrence depends more on correct technique for the grade and on diet and bowel habits afterwards than on the method alone.
