Blood in Stool: Is It Piles or Something Serious? (Latrine Mein Khoon)

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

Seeing blood in the toilet is frightening, and the first worry for many patients is cancer. The good news is that most rectal bleeding is caused by piles or a fissure. But because a few cases have a more serious cause, bleeding should always be examined — not guessed.

Quick answer: bright red blood on the paper or dripping after stool is most often from piles or a fissure. Dark, maroon or black blood, blood mixed within the stool, weight loss, anaemia or a lasting change in bowel habit are warning signs that need prompt examination and sometimes a colonoscopy.

Common causes of blood in stool

CauseType of bleedingPain?
Piles (bawaseer)Bright red, drips or on paperUsually painless
Anal fissureBright red streak on stool or paperSharp pain during/after stool
PolypsOften none, sometimes streaksNo
Colitis / infectionBlood with mucus, loose motionsCramps
Bowel cancerDark or mixed blood, sometimes hiddenOften painless early

Clues that it is likely piles

  • Bright red blood, separate from the stool.
  • Happens with straining or constipation.
  • No weight loss, normal appetite, no change in bowel pattern.

Red-flag signs that need prompt checking

  • Dark red, maroon or black stools.
  • Blood mixed through the stool rather than on the surface.
  • Unexplained weight loss, tiredness or anaemia (low haemoglobin).
  • A change in bowel habit — new constipation or loose motions — lasting more than 3–4 weeks.
  • Age above 45–50, or a family history of bowel cancer.
  • A lump in the abdomen, or bleeding that continues despite piles treatment.
Can piles turn into cancer? No. Piles do not become cancer. The real risk is different: a patient assumes all bleeding is "just piles" and delays the check-up that would have found another cause early.

What tests might be needed?

Most patients need only a clinical examination and proctoscopy in the OPD. If there are red-flag signs, the surgeon may advise a blood test for haemoglobin and a colonoscopy — a camera test of the large bowel done under sedation. It is the most reliable way to rule out polyps and cancer.

What you can do today

  • Note the colour of blood, how often it happens and whether there is pain.
  • Avoid self-medicating with ointments for weeks without a diagnosis.
  • Book an examination — especially if any red flag above applies.

If it turns out to be piles or a fissure, modern laser treatment is quick and day-care. If something else is found, finding it early is what matters most.
