Piles During Pregnancy and After Delivery: What Is Safe?

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

Up to a third or more of women develop piles during pregnancy or soon after delivery. It is uncomfortable and often embarrassing, but in most cases it settles with simple, safe care — and surgery is rarely needed during pregnancy.

Quick answer: piles in pregnancy happen due to pressure from the growing uterus, hormonal changes and constipation. Most improve with fibre, fluids, sitz baths and doctor-approved creams, and many settle within weeks after delivery. Procedures are usually deferred until after the baby is born.

Why are piles so common in pregnancy?

  • The growing uterus presses on veins in the pelvis and rectum.
  • Pregnancy hormones slow down the bowel, causing constipation.
  • Iron tablets can make stools harder.
  • Pushing during normal delivery strains the area further.

Safe relief during pregnancy

  • Fibre and fluids: whole grains, dal, fruits like papaya (ripe), guava and pear, vegetables, and 8–10 glasses of water unless your gynaecologist has restricted fluids.
  • Don't strain or sit long on the toilet — and leave the phone outside.
  • Warm sitz bath for 10–15 minutes, 2–3 times a day. See our sitz bath guide.
  • Lie on your left side when resting to reduce pressure on pelvic veins.
  • Stay gently active — walking helps the bowel.
  • Creams and stool softeners only after asking your doctor, because not every medicine is safe in pregnancy.
Do not start any over-the-counter tablet, "desi" churan or ayurvedic powder during pregnancy without asking your gynaecologist. Some contain strong laxatives or ingredients not tested for pregnancy.

After delivery

Piles often feel worse in the first days after a normal delivery, then steadily improve over 4–8 weeks as hormones settle and pressure is relieved. Keep stools soft (breastfeeding mothers need extra fluids), continue sitz baths, and do not hold back stool out of fear of pain.

A fissure is also common after delivery — if you have sharp, cutting pain every time you pass stool, read fissure after delivery and in women.

When is treatment needed?

See a surgeon promptly for heavy bleeding, a very painful hard lump (a clotted pile), or piles that come out and do not go back. During pregnancy, the surgeon will usually control symptoms and plan any procedure for a few months after delivery. If piles persist 3 months after delivery, laser treatment is a quick day-care option.
