Piles in Your 20s and 30s: Why Young Indians Are Getting Piles

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

A growing share of patients coming to the OPD with piles and fissures are in their 20s and 30s — software professionals, students preparing for exams, delivery riders and young parents. Lifestyle, not age, is the main driver.

Quick answer: young people get piles mainly because of long sitting hours, low-fibre fast food, too little water, irregular sleep and meal times, long toilet sitting with phones, and straining during heavy weightlifting. Most cases can be prevented — and early piles treated simply.

Why piles are rising in young adults

  • Long sitting: 9–12 hours at a desk or on a bike, with few breaks.
  • Low-fibre diet: maida, fast food, instant noodles, few vegetables and fruits.
  • Too little water and too much tea/coffee.
  • Irregular routine: late nights, skipped breakfast, ignoring the morning urge.
  • Phone on the toilet: long sitting and silent straining — see phone in toilet and piles.
  • Heavy gym lifting with breath-holding, which raises pressure in the abdomen.
  • Protein-heavy diets with few vegetables, causing hard stools.

Desk-worker prevention plan

  1. Stand up and walk for 2–3 minutes every 45–60 minutes.
  2. Keep a 1-litre water bottle on the desk and finish it twice during work hours.
  3. Add fibre at every meal — a fruit, a salad or sabzi, and whole-wheat roti or daliya.
  4. Keep a fixed time for the toilet, usually after breakfast; don't delay the urge because of meetings.
  5. At the gym, breathe out during the lift and avoid maximal lifts while you have symptoms.
A cushion alone doesn't prevent piles. Movement, fibre, water and good toilet habits do.

"I'm too young for piles — it must be something else?" "इतनी कम उम्र में बवासीर?"

Young patients are often embarrassed and delay the check-up for months, self-treating with creams. Many actually have a fissure, which needs different care. A short examination gives the correct diagnosis, and early piles usually settle without surgery.

When young patients need a procedure

If piles are grade 3–4, bleed repeatedly or keep flaring up, laser treatment suits working people well — it is usually day-care, with most back at a desk job within a few days.

Frequently asked questions

Can piles happen at 20 or 25?+
Yes. Piles are increasingly common in young adults, mainly due to long sitting, low-fibre diets, dehydration and poor toilet habits.
Does going to the gym cause piles?+
Heavy lifting with breath-holding raises abdominal pressure and can worsen piles. Breathing out during the lift and avoiding maximal lifts during a flare helps.
Can a desk job cause piles?+
Long uninterrupted sitting is a known risk factor. Regular breaks, walking, water and fibre reduce the risk considerably.
How soon can I return to work after laser piles treatment?+
Most people with desk jobs return within 2–4 days, depending on the extent of treatment.
