Anal Fissure: Symptoms, Causes and Acute vs Chronic (Fissure Kya Hai)

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

If passing stool feels like passing glass, followed by burning that lasts minutes to hours, you likely have an anal fissure — a small tear in the lining of the anus. It is one of the most painful common conditions, yet one of the most treatable.

Quick answer: an anal fissure is a small tear in the anal lining, usually caused by passing a hard stool or by repeated diarrhoea. The typical symptoms are sharp pain during stool, burning pain lasting minutes to hours afterwards, and a little bright red blood. A fissure present for more than 6–8 weeks is called chronic.

Typical symptoms

  • Sharp, cutting pain while passing stool.
  • Burning or throbbing pain afterwards, often lasting 30 minutes to a few hours.
  • A streak of bright red blood on the stool or toilet paper.
  • Fear of passing stool, leading to holding back — which makes stools harder.
  • A small skin tag at the anal edge in long-standing fissures.

Why does the pain last so long?

The tear irritates the internal sphincter muscle, which goes into spasm. The spasm causes pain and also reduces blood flow to the tear, so it heals slowly — a vicious cycle. Treatment aims to relax that muscle and keep stools soft.

Common causes

  • Repeated diarrhoea.
  • Straining, especially with long toilet sitting.
  • Childbirth — fissures are common after delivery.
  • Low-fibre diet, too little water.

Fissure or piles?

Piles usually bleed without pain; a fissure hurts a lot and bleeds a little. Many patients call every anal problem "piles" and use the wrong treatment. See the full piles, fissure or fistula comparison.

Unusual fissures need a closer look. Most fissures are at the back (6 o'clock position). Multiple fissures, fissures at the sides, or those that don't heal despite treatment may need tests to rule out other conditions such as inflammatory bowel disease or infection.
