A fissure is a small tear in the lining of the anus, usually caused by passing a hard stool, repeated diarrhoea or childbirth. The tear irritates the internal sphincter muscle, which goes into spasm — causing intense pain and reducing blood flow so the tear heals slowly.
Fissures under 6 weeks are acute; beyond 6–8 weeks they are chronic, often with a small skin tag. Read fissure symptoms and causes.
See a doctor urgently if: swelling with fever or pus (possible abscess), heavy bleeding, or multiple or unusual fissures that don't heal.
| Option | Best for | Notes |
|---|---|---|
| Soft stools, sitz baths, prescribed ointments | Acute fissures; first step for chronic | 6–8 weeks of regular use |
| Botox injection | Selected chronic fissures | Temporary muscle relaxation |
| Lateral internal sphincterotomy (LIS) | Chronic fissure not healing with medicines | Gold-standard; high healing rate |
| Laser fissure treatment | Fissure bed, skin tag, associated piles | Minimal pain; often combined |
Muscle tone, previous deliveries and bowel habits are assessed before choosing, to protect continence. See laser vs sphincterotomy and incontinence risk explained.

Many fissures are treated with medicines alone at OPD cost. When a procedure is needed, cost depends on the technique, whether piles or tags are treated at the same time, anaesthesia and room category. A clear estimate is given after examination.
Fissure surgery is generally covered by health insurance after the policy waiting period. Read insurance coverage.
