Anal Fissure in Children: Painful Potty, Blood and Constipation

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

A child who cries during potty, hides, or holds back stool — and then there's a little blood on the stool — very often has an anal fissure. It's common, it's treatable, and it rarely needs surgery in children.

Quick answer: in children, fissures are almost always caused by constipation and hard stools. Treating constipation is the treatment: fluids, fibre, regular toilet time, warm baths and a child-safe stool softener if the doctor advises. Most heal within a few weeks.

Signs parents notice

  • Crying, straining or refusing to sit on the potty.
  • A streak of bright red blood on the stool or wipe.
  • Holding stool — crossing legs, standing on toes, hiding in a corner.
  • Hard, large or pellet-like stools, or stool going only every 3–4 days.

The withholding cycle

Hard stool → tear and pain → child is afraid → holds stool → stool gets harder → more pain. Breaking this cycle is the whole treatment.

What parents can do

  1. More fluids — water, milk in the right amount (too much milk can constipate), soups, dal water.
  2. Fibre the child enjoys — fruits like papaya, pear, banana (ripe), guava; vegetables in paratha or khichdi; whole-wheat roti.
  3. Fixed toilet time — 5 minutes after breakfast and dinner, with feet supported on a stool.
  4. Warm bath or sitz bath after potty to ease pain.
  5. Praise, never scold for accidents or refusing.
  6. Stool softeners — only the one your paediatrician or surgeon prescribes, for as long as advised.
Limit junk: chips, biscuits, maggi and too much milk with few vegetables are common reasons children in cities become constipated.

When to see a doctor

  • Bleeding that is more than a streak, or continues after 2 weeks of home care.
  • Swelling, fever or pus near the anus.
  • Constipation since the first weeks of life, poor weight gain or a swollen tummy.
  • Fissures that keep returning.

Surgery is rarely needed in children. For adults, read anal fissure symptoms and causes.
