Fissure /
How Long Does an Anal Fissure Take to Heal Without Surgery?
The good news about a fresh fissure is that most heal without any operation. The key is breaking the cycle of hard stool, pain and muscle spasm early — and giving the tear the right conditions to heal.
Quick answer: most acute fissures heal within 4–6 weeks with soft stools, sitz baths and doctor-prescribed ointments. Chronic fissures (more than 6–8 weeks) heal with medicines in roughly half of cases; if they don't, a short procedure usually gives a lasting cure.
Typical healing timeline
| Time | What usually happens |
|---|---|
| Week 1 | Pain during stool starts easing once stools are soft and sitz baths begin |
| Week 2–3 | Burning after stool shortens; bleeding reduces |
| Week 4–6 | Most acute fissures are healed |
| Beyond 6–8 weeks | Fissure is chronic — review treatment; consider a procedure if not improving |
What helps a fissure heal
- Soft stools every day — fibre, 2.5–3 litres of water, and isabgol if advised.
- Warm sitz baths 2–3 times a day and after stool — they relax the spasm.
- Prescribed ointments that relax the sphincter muscle, used regularly for 6–8 weeks. See do fissure creams work.
- Don't hold back stool out of fear — it only makes the next one harder.
- Short toilet time, no straining, no phone.
What slows healing
- Stopping the ointment as soon as pain eases (the tear is not yet healed).
- Returning to maida, fried food and low water.
- Repeated diarrhoea from outside food.
- Using harsh creams or home remedies that irritate the skin.
Healed doesn't mean immune. A fissure can tear again with one hard stool. Keep the fibre-and-water habit for at least a few months after healing.
When is surgery the better option?
Consider a procedure if the fissure has lasted more than 8 weeks despite proper treatment, keeps coming back, or has a skin tag and raised edges. The standard operation — lateral internal sphincterotomy — has high healing rates, and recovery is usually quick. Read fissure surgery options.

Frequently asked questions
Can a fissure heal on its own?+
Many acute fissures heal on their own within 4–6 weeks if stools are kept soft and straining is avoided.
Why is my fissure not healing?+
Common reasons are ongoing hard stools or diarrhoea, stopping ointments too early, and a chronic fissure with muscle spasm that needs a procedure.
How long should I use fissure ointment?+
Usually 6–8 weeks, as prescribed, even after pain improves.
Can a healed fissure come back?+
Yes, a hard stool can tear the lining again. Continuing a fibre-rich diet and good hydration reduces this risk.
