Sitz Bath for Piles and Fissure: How to Do It at Home

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

A sitz bath is one of the simplest and most effective things you can do at home for piles, fissure, and recovery after anal surgery. It relaxes the muscle around the anus, improves blood flow and soothes pain and itching. Here is how to do it properly.

Quick answer: sit in comfortably warm (not hot) plain water covering the anal area for 10–15 minutes, 2–3 times a day and after every stool. Pat dry gently afterwards. Salt, antiseptics or herbs are not needed unless your doctor advises.

What you need

  • A clean plastic tub (a wide "tasla" or a sitz bath basin that fits on the toilet seat).
  • Warm water — comfortably warm to your wrist, not hot.
  • A soft, clean towel.

Step by step

  1. Clean the tub and fill it with warm water about 3–4 inches deep.
  2. Test the temperature with your wrist or elbow — it should feel pleasantly warm.
  3. Sit in the water so the anal area is fully covered. Keep knees relaxed.
  4. Stay for 10–15 minutes. Add a little warm water if it cools.10–15
  5. Get up, and gently pat the area dry — don't rub.
  6. Apply any prescribed ointment after drying.

How often?

2–3 times a day, and ideally after each bowel movement. After surgery, your surgeon may advise it more often in the first week.

Why it works: warm water relaxes the internal sphincter muscle. In a fissure, this muscle is in spasm and causes most of the pain — so a sitz bath gives real relief, not just comfort.

Common mistakes

  • Water too hot — it can burn the delicate skin.
  • Adding lots of salt, Dettol or potassium permanganate — these can irritate the skin; plain water works.
  • Sitting for 30–40 minutes — longer is not better and can cause swelling.
  • Rubbing dry with a rough towel.
  • Using it instead of treatment for a long-standing problem.

Hot or cold?

Warm water is standard for pain and muscle spasm. A cold pack wrapped in cloth for a few minutes can help a very swollen external pile — but for most people, warm is the right choice.

A sitz bath is a support, not a cure. If symptoms persist beyond 2–3 weeks, read about fissure healing time or piles symptoms and get examined.
