Piles Symptoms: 7 Early Signs You Should Not Ignore (Bawaseer ke Lakshan)

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

Piles (bawaseer, hemorrhoids) usually start quietly — a few drops of blood on the toilet paper, some itching, a feeling that the bowel has not emptied fully. Because the early signs are mild, many people ignore them for months. Catching piles early usually means simpler treatment.

Quick answer: the commonest early signs of piles are painless bright red bleeding during stool, itching or irritation around the anus, a soft lump that may come out while passing stool, and a feeling of incomplete emptying. Any rectal bleeding should be checked once by a doctor.

The 7 early signs of piles

  1. Bright red blood during or after stool — on the paper, dripping into the pan, or streaking the stool. Usually painless.
  2. Itching or irritation around the anus, especially after stool or at night.
  3. A soft lump that comes out while straining and goes back on its own or has to be pushed in.
  4. Mucus or wetness that stains the underwear.
  5. Feeling of incomplete emptying — wanting to go again right after passing stool.
  6. Heaviness or discomfort while sitting for long hours.
  7. Sudden severe pain with a hard, bluish lump — this is a clotted (thrombosed) pile and needs early review.

Internal vs external piles

Internal piles form inside the anal canal. They usually bleed but do not hurt, because that area has few pain nerves. External piles form under the skin at the anal opening; they are more likely to itch, swell and hurt, especially if a clot forms. Many people have both.

Symptoms that are NOT typical of piles

Some symptoms point to another problem and should never be self-treated as piles:

  • Sharp, cutting pain during stool that lasts minutes to hours — this is more typical of a fissure.
  • A small boil or opening near the anus that leaks pus — this suggests a fistula.
  • Dark or maroon blood, blood mixed in the stool, weight loss, or a change in bowel habit lasting weeks — this needs examination to rule out other bowel conditions.
Important: piles are common, but bleeding from the back passage is never something to diagnose yourself. One proper examination — which takes only a few minutes in the OPD — settles the question.

When should you see a doctor?

  • Bleeding that happens more than once or twice.
  • A lump that does not go back in, or has to be pushed in.
  • Pain, swelling or a hard lump.
  • Symptoms that keep returning despite diet changes.
  • You are above 40, or have a family history of bowel disease.

What the doctor checks

The examination is short and private. The surgeon looks at the area and usually does a gentle finger examination and sometimes a proctoscopy (a small tube with a light) to see the piles and grade them from 1 to 4. The grade decides whether diet and medicines will be enough or whether laser treatment is the better option.

Frequently asked questions

What is the first sign of piles?+
The most common first sign is painless bright red bleeding during or after passing stool, often noticed on toilet paper.
Do piles always hurt?+
No. Internal piles usually bleed without pain. Pain is more common with external or clotted piles, or when a fissure is also present.
Can piles symptoms come and go?+
Yes. Piles often flare up with constipation, long sitting or spicy, low-fibre meals and settle in between — but they tend to return if the cause is not treated.
Is every rectal bleeding piles?+
No. Fissure, polyps, inflammation and, rarely, cancer can also cause bleeding. That is why any repeated bleeding should be examined once by a doctor.
