Do Fissure Creams and Ointments Really Work? What Patients Should Know
Many patients buy a cream from the chemist, use it for a few days, and give up. Prescribed fissure ointments do work for a large number of patients — but only when the right type is used correctly for long enough.
Quick answer: prescribed fissure ointments (such as nitrate or calcium-channel-blocker preparations) relax the spasm of the anal muscle and improve blood flow so the tear can heal. Used regularly for 6–8 weeks, they heal many acute and about half of chronic fissures. Numbing gels only relieve pain; they do not heal.
Types of fissure ointments
| Type | What it does | Notes |
|---|---|---|
| Muscle-relaxing ointments (nitrate or calcium-channel-blocker based) | Relax the spasm, improve blood flow, help healing | Main treatment; used for 6–8 weeks |
| Numbing gels (local anaesthetic) | Reduce pain during stool | Comfort only, don't heal |
| Soothing / barrier creams | Protect skin | Supportive |
| Steroid creams | Reduce itching/inflammation | Not for long-term use on fissure |
How to use them correctly
- Use only the ointment your doctor prescribes, in the amount and frequency advised.
- Apply after a sitz bath and drying, usually 2–3 times a day.
- Continue for the full course (often 6–8 weeks), even if pain improves in a week.
- Keep stools soft throughout — cream can't heal a tear that keeps reopening.
Side effects to know साइड इफ़ेक्ट
- Headache is common with nitrate-based ointments, especially in the first days; it often reduces over time. Tell your doctor if it's troublesome — there are alternatives.
- Dizziness on standing in some people.
- Some ointments are not suitable with certain heart medicines — always tell the doctor what you take.
Never use a leftover steroid cream for weeks on a fissure. Long steroid use can thin the skin and slow healing.
When creams are not enough
If a fissure has not healed after 6–8 weeks of proper use, has become chronic with a skin tag, or keeps coming back, a procedure usually gives a lasting cure. Options include Botox injection (temporary muscle relaxation) and lateral internal sphincterotomy. See laser vs sphincterotomy.
