Fissure Surgery Options: Laser vs Lateral Internal Sphincterotomy

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

When a chronic fissure refuses to heal, a short procedure can end months of pain. Patients often ask whether "laser" or "cut" surgery is better. Here is an honest comparison.

Quick answer: lateral internal sphincterotomy (LIS) is the most proven operation for chronic fissure, with high healing rates. Laser can be used to treat the fissure bed, skin tag and associated piles with minimal pain, and is often combined with sphincter relaxation. The best plan depends on the fissure, muscle tone and any other problems present.

Who needs a procedure?

  • Chronic fissure (more than 6–8 weeks) not healing with ointments.
  • Fissures that keep coming back.
  • Fissure with a skin tag, papilla or associated piles.

The options

Lateral internal sphincterotomy (LIS)

A small, precise cut is made in part of the internal sphincter muscle at the side to permanently relieve the spasm. The fissure then heals on its own. It is considered the gold-standard operation.

Laser treatment

Laser energy cleans and treats the fissure bed and removes the skin tag or papilla with very little bleeding and pain. Depending on muscle tone, the surgeon may combine it with sphincter relaxation.

Botox injection

A small injection temporarily relaxes the sphincter for a few months, allowing healing. Useful for selected patients, though recurrence is higher than with LIS. ||

What about leakage (incontinence)?

This is the most common worry. With a carefully measured sphincterotomy in suitable patients, the risk of long-term problems is low and usually limited to minor difficulty controlling gas. Women who've had several deliveries, older patients and those with weak muscle tone need a tailored approach. Read fissure surgery and incontinence risk.

The operation matters less than the assessment. Checking muscle tone, looking for associated piles or a hidden fistula, and planning for your body gives the best result.

Recovery is usually quick: soft diet, sitz baths and softeners for 2–3 weeks. See our Fissure Doctor in Indore page for how treatment is planned.

Frequently asked questions

Is fissure surgery painful?+
Most patients feel much less pain after surgery than they did with the fissure itself. Discomfort is mild and controlled with tablets.
Which is better for fissure — laser or LIS?+
LIS has the strongest evidence for chronic fissure. Laser is useful for the fissure bed, tags and associated piles, often in combination. The best choice depends on your examination.
Can fissure come back after surgery?+
Recurrence after a properly done sphincterotomy is uncommon; keeping stools soft reduces risk further.
How many days rest after fissure surgery?+
Most people return to desk work within 2–5 days.
