Silent Gallstones: Stones on Ultrasound but No Pain — Do You Need Surgery?

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

Many people discover gallstones by chance — during a routine health check-up or an ultrasound for something else. They've never had pain. Should they have surgery? For many, the answer is no — but there are important exceptions.

Quick answer: most people with truly silent gallstones can safely be monitored, because many never develop symptoms. Surgery is usually advised if pain or complications develop, or if there are risk factors such as large stones (about 3 cm or more), a calcified (porcelain) gallbladder, a polyp of 1 cm or more, or a thickened gallbladder wall.

What happens to silent stones? बिना दर्द वाली पथरी का क्या होता है

Most people with silent gallstones stay symptom-free for years. A small proportion each year develop pain, and a smaller number develop complications. The first sign is usually a pain attack rather than a sudden complication.

The Indian context भारतीय संदर्भ

Parts of North and Northeast India have some of the world's highest rates of gallbladder cancer, and gallstones are its main risk factor. The absolute risk for any one person remains low, but it's one reason Indian surgeons discuss earlier surgery more readily in certain situations. Read gallstones and gallbladder cancer risk.

If you choose to watch: learn the warning signs — pain in the upper right or middle abdomen after meals, pain going to the right shoulder or back, fever, yellow eyes or dark urine — and see a surgeon promptly if they appear.

Common symptoms people don't link to stones लक्षण जिन्हें लोग पथरी से नहीं जोड़ते

Heaviness after fatty meals, recurrent "acidity" at night, or upper abdominal pain lasting 30 minutes to hours may actually be biliary pain. See gallstone pain vs acidity. If you've had these, your stones may not be silent after all.
