Do Gallstones Cause Gallbladder Cancer? What Indian Patients Should Know

By Dr. Bhupendraa PrrasadBombay Hospital, IndoreReviewed Sep 20263 min read

Patients in India often ask whether their gallstones could turn into cancer. It's a fair question: parts of India have some of the world's highest rates of gallbladder cancer. But the risk needs to be understood in proportion.

Quick answer: gallstones are the strongest known risk factor for gallbladder cancer — most patients with this cancer have stones — but the vast majority of people with gallstones never develop cancer. The risk is higher with large stones, long-standing stones, a porcelain gallbladder, polyps of 1 cm or more and a thickened wall. Parts of North and Northeast India have especially high rates.

Understanding the risk

  • Most people with gallbladder cancer have gallstones.
  • But most people with gallstones will never get cancer.
  • Long-term irritation of the gallbladder wall by stones is thought to play a role.

Where in India is it more common?

Gallbladder cancer is notably more common along the Gangetic plains — parts of Uttar Pradesh, Bihar, West Bengal — and in Assam and the Northeast. Women are affected more than men. Diet, water, genetic and environmental factors are all being studied.

Features that raise concern

  • Stones 3 cm or larger.3
  • Porcelain gallbladder (calcified wall).
  • Polyp 10 mm or more, or a growing polyp — see gallbladder polyp.
  • Irregular or thickened wall on ultrasound.
  • Stones for many years with repeated inflammation.

Symptoms to never ignore

Early gallbladder cancer usually has no symptoms. Later signs include persistent upper right pain, unexplained weight loss, loss of appetite, a lump in the upper abdomen, and jaundice (yellow eyes) without fever. These need urgent evaluation with scans.

A practical approach: if you have symptomatic gallstones, timely laparoscopic removal treats the pain and removes this long-term risk. If your stones are silent, discuss the scan features above with a surgeon rather than simply ignoring them.

Every removed gallbladder is examined under the microscope (histopathology), which occasionally detects early cancer that can then be managed at a curable stage. Read silent gallstones.
