Weight-Loss Injections, Crash Diets and Gallstones: What You Should Know
Weight-loss medicines such as semaglutide and tirzepatide are being used by more and more people in India. They can be very effective. One side effect doctors watch for is gallbladder problems — especially gallstones — which are linked to rapid weight loss of any kind.
Quick answer: rapid weight loss — from GLP-1 weight-loss injections, very-low-calorie crash diets, long fasting or bariatric surgery — increases the risk of gallstones and gallbladder inflammation. Losing weight steadily under medical supervision, not skipping meals completely and watching for warning symptoms lowers the risk.
Why fast weight loss causes gallstones
- When fat is broken down quickly, the liver releases more cholesterol into bile.
- Eating very little means the gallbladder empties less often, so bile sits and thickens (sludge).
- Some weight-loss medicines also slow gallbladder emptying.
Who is at higher risk
- Losing more than about 1–1.5 kg per week for many weeks.
- Very-low-calorie or crash diets, long fasting.
- After bariatric (weight-loss) surgery.
- Women, people over 40, and those with existing gallbladder sludge or stones.
Warning symptoms
- Pain in the upper right or upper middle abdomen, especially after meals.
- Pain spreading to the right shoulder or back.
- Fever, yellow eyes, dark urine.
- Severe upper pain with vomiting — possible pancreatitis.
Don't stop or start weight-loss medicines on your own. If you have these symptoms while on treatment, contact the prescribing doctor and get an ultrasound.
How to lower the risk
- Aim for steady weight loss agreed with your doctor.
- Don't skip meals entirely — include some healthy fat and protein in regular meals, which helps the gallbladder empty.
- Stay active and well hydrated.
- After bariatric surgery, your surgeon may prescribe a bile-acid medicine for some months to reduce stone formation.
- If you already have symptomatic gallstones, discuss gallbladder removal before or alongside a weight-loss plan.

Frequently asked questions
Can semaglutide or weight-loss injections cause gallstones?+
Gallbladder problems, including gallstones, are a recognised side effect linked partly to rapid weight loss. Discuss your risk with the prescribing doctor.
How fast is too fast for weight loss?+
Sustained loss of more than about 1–1.5 kg per week raises gallstone risk. Steady, supervised loss is safer.
Should I remove my gallbladder before weight-loss surgery?+
If you have symptomatic gallstones, your surgeon may advise removal before or during weight-loss surgery. Silent stones are assessed case by case.
Does intermittent fasting cause gallstones?+
Very long fasting periods can reduce gallbladder emptying. Moderate, regular eating patterns are safer for people prone to stones.
